Navigating the Referral & Intake Queue

A quick reference guide on how to navigate the referral & intake queue tab on left side navigation in https://care.cordatahealth.com

These queues are located on the left hand navigation on your home page. They help you determine which patients need a particular action.

NEW REFERRALS

- Patients get into this queue in 2 different ways
1. Using the "Add new Inbound Referral" button

2. Patients connected to Cordata through an interface that is configured to populate the New Referrals queue
- A Patient in the New Referrals queue has a checklist that has not been touched, no item is in progress or complete on the checklist. All items in the Intake Checklist are "Not Started"

- Once an item in the checklist is started, the patient will move OUT of the NEW REFERRALS queue and INTO the PENDING INTAKES queue.

PENDING INTAKES

- As soon as an item in the INTAKE CHECKLIST is "In Progress" the patient will move into the PENDING INTAKES queue.
- A patient will stay in this queue until the INTAKE CHECKLIST has all items checked off and the COMPLETE button is clicked.

- Once the COMPLETE button is clicked, the patient will move to the either the READY FOR REVIEW queue or the PENDING FILMS.

PENDING FILMS

- If there are films that have a status of "AWAITING RESULTS" the patient will show up in the PENDING FILMS queue.
- Films can be entered in 2 places
1. Medical History - when entering in this location, the status will automatically be AWAITING RESULTS unless PACS is selected as the Image location.

This will force this patient to go to the PENDING FILMS queue until the status of this test is changed.
To change the status of a test, first click into the patient chart, then click Diagnostic Tests under Clinical Interventions.

Click into the test to change the result. Click Edit. Now you can change the results.



2. Click directly into Diagnostic Tests from the left hand navigation under Clinical Interventions.

Click New Diagnostic Test

Enter test information. If you would like the test to show up in the PENDING FILMS queue, you must have the status as AWAITING RESULTS and the location must be something other than PACS.

Once ALL films have a status out of AWAITING RESULTS and the FILMS ACQUIRED checkbox is checked on the checklist, the patient will move to the READY FOR REVIEW queue.

READY FOR REVIEW

This queue is where patients who are ready to review stay. Ready for review means all films have been checked in and the intake checklist is complete; however the patient has not been sent to review yet.

Once a patient is sent to review, the patient will move from READY FOR REVIEW to PENDING REVIEWS.



PENDING REVIEWS

This queue is where patients that have been sent to review, but the review has not been completed yet stay. Once the review is completed, the patient will move out of this queue and into another.
